Appal: Discount & Free Gift

This app lets you add free gifts to customer carts when they hit a spend or quantity threshold. The gift automatically adds at zero cost and removes if the cart drops below the threshold. You can set up tiered discounts so larger carts earn better gifts, or let customers choose from a list you create. Different thresholds can be set per market. All offers run on Shopify's native discount engine, so they work in the cart and at checkout without requiring a discount code.
