Professional Form Manager helps you create custom storefront forms with file uploads and get submissions delivered by email. Upload your fillable PDF — like an application, agreement, or order form — and the app automatically builds a matching form for your store, then fills your PDF template with each submission. Forms can also generate PDFs automatically without a template. Build forms with conditional logic, repeatable sections, and file upload fields. Each submission goes to your team by email with files included, plus customers get an auto-reply. Review all submissions in your admin with sensitive fields masked for privacy, export data to CSV, and rely on built-in spam checks to keep your forms secure.
